Family members, carers and authorised representatives often help start support conversations. The most useful enquiries explain who needs support, what is being requested and who should be contacted.
What to include
- Your relationship to the person.
- The support pathway, if known.
- The person's suburb and preferred contact details.
- The support tasks or goals being discussed.
- Whether consent or authority to discuss support is in place.
When the pathway is unclear
If you are not sure whether the enquiry relates to NDIS or Support at Home, describe the person's situation in general terms and ask which pathway is appropriate for the first discussion.
