Before you start
- Confirm the participant name and preferred contact details.
- List the supports being requested, such as personal care, transport or community participation.
- Include the suburb or general service area.
- Provide referrer details and consent to be contacted.
Choosing the correct pathway
Use the NDIS pathway when the enquiry is for an NDIS participant. Support at Home enquiries use a separate pathway because the provider arrangements are different.
